Question For Breastfeeding Mothers, Please Help!?!?

I have had engorgement issues from day one. I am a first time mommy, my daughter is 2 weeks old. The day I got home from the hospital, I was so engorged that my baby would not latch. I bought a pump, so I decided to pump that night to relieve some engorgement. The pump RUINED my nipples, made them bleed, crack, and tore holes in them. It wad awful. I immediately made an appointment with the Lactation service at the hospital, and she suggested to rent a hospital grade pump, and to exclusively pump until my nipples healed.
So I did. I supplemented formula and breast milk and have for the passed week ans a half. Anyways, I was not aware that in order to keep your milk supply up, you must pump between 6-8 times a day- I only pumped once at night. So obviously my milk supply has gone down tremendously.
Now im trying to get my supply back up- im pumping 8 times a day with the hospital pump, and my milk is slowly going up. My questions are these: Once I get my supply back to normal, is it possible to get her to latch again, to my breast? I tried, and the Lactation lady said it seems my daughter has grown VERY accustomed to the rubber nipple of the bottle. Also, since im still working on my milk supply, I am still supplementing formula and breast milk. Can I give my baby pure breast milk in one feeding then do a formula only feeding the next, or should I mix them?
